Functional Medicine Utilizes Detailed Information Gathering and Advanced Diagnostic Testing

Functional medicine providers gather information from genetics, labs, and lifestyle practices to gain a full understanding of your health. All new patients are required to complete in-depth medical and family history questionnaires, and our providers depend on advanced testing to assess the status of many functions within your body (microbiome, adrenal function, hormonal health, nutritional deficiencies, heavy metal toxicities, food sensitivities, amino acids, or more). This combination of information helps guide the selection of the best therapies to help you achieve optimal health.  

Functional Medicine Emphasizes Lifestyle Treatments

Functional medicine emphasizes lifestyle modifications in the treatment of chronic illness. Factors like nutrition, sleep, connection, physical activity, and stress management are intricately connected and examining them in-depth helps providers gain comprehensive understanding of a person’s health and begin to establish a plan of care that positively impacts all 5 areas. 

Functional Medicine Provides In-Depth Patient Education

Functional medicine provides extensive patient education on a wide variety of medical, nutritional, and lifestyle topics. Shared medical visits (SMVs) are hour-long sessions where patients are seen by a provider, nutritionist, or health coach in an educational, interactive, group setting. Shared medical visits can vary in size, but no more than 10 patients will be in a visit. 

Becoming a Patient at FMI

Step #1: New Patient Inquiry

To start the process of becoming a patient with Functional Medicine of Idaho, please complete the New Patient Inquiry Form below. After submitting the form, you will receive a New Patient Inquiry Call from an FMI Patient Experience Navigator.

During the New Patient Inquiry Call, a Patient Experience Navigator will discuss our patient process, insurance coverage, billing questions, and answer any questions you may have.

Step #2: Patient Account Registration

Functional Medicine of Idaho uses an online portal to track all patient records, appointments, and communications. Immediately after your New Patient Inquiry Call, you will receive an email containing the link to finish setting up your account in the Patient Portal. 

Once you’ve logged in to your patient portal, please complete all intake questionnaires located under “Questionnaires”, and upload an image of the patient and insurance card under “Labs & Documents”.

Step #3: Scheduling Call

Once you’ve submitted all intake forms in the patient portal, you will receive a Scheduling Call from an FMI Patient Experience Navigator. 

During this Scheduling Call, a Patient Experience Navigator will schedule your Provider Intake Visit, address any missing information in your patient portal, and answer any questions you have.

Accepted Insurances

Our team of providers is in network with most Blue Cross, Regence, Aetna, Select Health, Pacific Source, Cigna, and Mountain Health Co-op plans.

Out of Network Information:
Most insurances not listed above are considered out-of-network, including: Medicare, Medicaid, etc. For out-of-network patients, we offer a 15% cash discount for appointments. Additionally, we can provide you with a detailed bill for most services that contain ICD-10 diagnostic codes and CPT codes for reimbursement. We accept all HSA and FSA cards as a form of payment.
