To start the process of becoming a patient with Functional Medicine of Idaho, please complete the inquiry form below.

After the submission of your inquiry you will be forwarded to our registration form to create your FMI patient portal. A link will also be emailed to you if you need to finish at a later time.

If you are interested in care for multiple members of your family, be sure to fill out an inquiry form for EACH person. Each person will need their own FMI Patient Portal as well.

Patient Intake Forms & Insurance Verification
Once you have created your secure and convenient FMI Patient Portal, please upload a copy of your insurance card (front and back), a form of payment (credit, debit or HSA card), a clear profile photo of yourself, and complete the following forms:

For Adult Medicine & Women’s Health, complete: HIPAA Policy, Lab Information, Medical History and Medical Symptom Questionnaire, and Readiness Assessment. 
*If you have MEDICARE, please sign the Medicare Private Contract. *

For Pediatric Care complete: HIPAA Policy, Financial Agreement, Functional Medicine History, and the Pediatric Symptom Review 1.

Conveniently Upload Insurance Card
To upload a copy of the insurance card, front and back, please click the tab labeled Labs & Documents. From there, you will be able to upload photos of your card.  Unloading your insurance card prior to scheduling allows our team to verify insurance and ensure we are in network with your specific plan. 

Out of Network Information:
Most insurances not listed above are considered out-of-network. Out of network patients are considered self pay although FMI can provide you with a detailed bill for most services that contain ICD-10 diagnostic codes and CPT codes for reimbursement from your insurance. We accept all HSA and FSA cards as a form of payment.

Medicare & Medicaid: Functional Medicine of Idaho is NOT contracted with Medicare and Medicaid.
Medicare and Medicaid patients are consider self-pay patients.
It’s important to note that Medicare and Medicaid will not reimburse for care at FMI.

Scheduling Call

Once you’ve submitted all intake forms in the patient portal and uploaded your insurance for verification, you will receive a Scheduling Call from an FMI Care Coordinator to schedule your initial consultation and begin your journey to healing!

During this Scheduling Call, a Care Coordinator will address any missing information in your patient portal, and can answer any questions you have.
