Once registered, complete all of the intake questionnaires located on the right hand side of your patient portal. After these are completed, you will be contacted by one of our patient experience navigators to begin the scheduling process.

Please allow 48 business hours after finishing the forms to be contacted by our New Patient Navigator for a Welcome Call. If you prefer to set a time and date for your Welcome Call, click on the appointments tab in the Patient Portal and select a time and date that works for you.

Billing:

Our team of providers is in network with most Blue Cross, Regence, Aetna, Select Health, Pacific Source, Cigna, and Mountain Health Co-op plans. Additionally, our OB/GYN Dr. Hudson is also in network with most United and Tricare plans, and our Pediatrician Dr. Brus is also in network with Tricare.

Please call your insurance provider to guarantee coverage. Most insurances not listed above are considered out-of-network, including: Medicare, Medicaid, etc. For out-of-network patients, we offer a 15% cash discount for appointments. Additionally, we can provide you with a detailed bill for most services that contain ICD-10 diagnostic codes and CPT codes for reimbursement. We accept all HSA and FSA cards as a form of payment.

Prescription Refills:

Transfer patients that have never been seen at FMI before are able to get prescription refills before their appointment if we have the name of the prescription, dosage, and pharmacy, and the patient is scheduled as a new patient on the provider’s schedule. We cannot fulfill prescription requests if you do not have an appointment scheduled.
