New Patient Intake Process

  • Beginning Your Journey

To begin your journey with FMI, you will be contacted by one of our patient experience navigators. They will usher you through the next steps and be there to answer any questions you may have. Next, you will be scheduled for a virtual discovery consultation with one of our health coaches. This discovery will help us get to know you better, and provide insight for your future visits. We are so excited to embark on this journey with you!

  • Your First Appointment

During your initial appointment, your provider will hear your story and review your health profile with you to gather information. Your provider will spend time listening to dive deep into your health history, past health issues, and ongoing health concerns. This visit will set the foundation for your journey to optimal health! *If you choose Functional Medicine of Idaho for primary care, one of the next steps will be to schedule your wellness (yearly physical) visit.

  • Gathering More Information

After your first appointment, your provider will request lab work or specialty testing to provide us with more insights into what is going on in your body at the cellular level. This in-depth information will help us determine the root cause of your underlying health concerns. We offer advanced diagnostic testing to assess the microbiome, adrenal function, hormonal health, nutritional deficiencies, heavy metals, food sensitivities, amino acids, and more. These tests help us identify patterns and guide us in choosing the best therapies to help you achieve optimal health.

  • Nutritional Consultation

This is where the real work begins. During your initial nutrition consultation, you will work with one of our nutritionists to determine how foods can help restore balance and promote health. We’ll look at your eating patterns, and provide you with dietary recommendations, education, and support to modify behaviors and achieve optimum health through improved nutrition. Together, we will develop a customized plan of high-quality, nutrient-dense foods to address imbalances and foster the connection between nourishment and healing. Nutrition is the core modality of functional medicine and is the key to building foundation for optimal health.

  • Lifestyle Consultation

This is where the real transformation happens. During this appointment, we will reflect on your journey and work together to identify challenges, sources of inspiration and motivation that will empower you to make the necessary shifts towards optimal health. Together, with your coach, you will create a realistic plan that will help you accomplish your goals and make the lifestyle modifications necessary for lasting change and life-long health. These appointments will ensure you have the support, guidance, and information you need for success!

  • Shared Medical Visits

Shared medical visits will add another layer of support to your experience. Nutrition classes, group visits and intensives allow us to combine clinical care, health education, and peer support to better serve you. Bringing together a community of like-minded peers will allow  you to connect, support, challenge and cheer each other on towards similar goals. And the bonus? This additional support will improve your health outcomes!

  • Continued Individualized Care

From this point, we allow you to set the pace for the amount of check ins and follow ups with your care team. At Functional Medicine of Idaho, we love being a part of your health journey and are available for accountability, further education, and ongoing support.
