Foundational Lifestyle Program

Meridian and Boise Locations

As the demand for functional healthcare continues to grow in the Treasure Valley and throughout the state, our clinics have grown significantly. To meet this increasing need and ensure we are able to provide functional care to as many people as possible, we’ve introduced a Foundational Lifestyle Program for new patients, rooted in established functional medicine practices and designed to better align our patients with our unique mission while accelerating their journey to health.

Once you’ve submitted all 11 intake forms in the patient portal, you will receive a Welcome Call from an FMI Patient Experience Navigator within 5 business days.
