New Patient Process

Step #1: New Patient Inquiry

After submitting the New Patient Inquiry Form, you will receive a New Patient Inquiry Call from an FMI Patient Experience Navigator. 

During the New Patient Inquiry Call, a Patient Experience Navigator will discuss our patient process, insurance coverage, billing questions, and answer any questions you may have. 

Step #2: Patient Account Registration

Functional Medicine of Idaho uses an online portal to track all patient records, appointments, and communications. Immediately after your New Patient Inquiry Call, you will receive an email containing the link to finish setting up your account in the Patient Portal. 

Once you’ve logged in to your patient portal, please complete all intake questionnaires located under “Questionnaires”, and upload an image of the patient and insurance card under “Labs & Documents”.

Step #3: Scheduling Call

Once you’ve submitted all intake forms in the patient portal, you will receive a Scheduling Call from an FMI Patient Experience Navigator.

During this Scheduling Call, a Patient Experience Navigator will schedule your Provider Intake Visit, address any missing information in your patient portal, and answer any questions you have.

Step #4: Provider Intake Visit

Functional medicine supports the idea that you, along with your provider, are a co-creator of your health. During the Provider Intake Visit, you will meet with your provider, have the opportunity to discuss your health concerns, and formulate a treatment plan together. You can expect to receive evidence-based, functional healthcare that is rooted in the latest research in modern medicine and tailored to your individual needs.

What to Expect During Your Provider Intake Visit

This hour-long intake visit with your provider will include:

  • A discussion about your symptoms, concerns, and health goals
  • A review of your detailed medical history
  • The development of an in-depth treatment plan to address the root cause of your illness and help you reach your health goals


During your Scheduling Call (Step #3), a Patient Experience Navigator will help you schedule a Provider Intake Visit that works with your schedule.

Billing Information

Our in-network patients pay a copay for the Provider Intake Visit, and their insurance will be billed for CPT 99215 + any additional charges incurred during the appointment.

For out-of-network patients, we offer a 15% cash-pay discount for the Provider Intake Visit. We accept all HSA and FSA cards as a form of payment, and can provide you with a detailed receipt to submit directly to your insurance for reimbursement.
