To meet this increasing need and ensure we are able to provide functional care to as many people as possible, we’ve introduced a Foundational Lifestyle Program for new patients, rooted in established functional medicine practices and designed to better align our patients with our unique mission while accelerating their journey to health. 

New Patient Foundational Lifestyle Program

Functional medicine emphasizes nutrition and lifestyle treatments to address the underlying issues that lead to chronic illness. By participating in the Foundational Lifestyle Program, you can jump start your functional health journey by working with nutritionists and health coaches to address relevant modifiable lifestyle factors before meeting with your provider. This allows your Provider Intake Visit to be a more efficient and effective deep dive into your health concerns. 

All new adult patients at our Boise & Meridian locations have the opportunity to participate in the Foundational Lifestyle Program prior to their Provider Intake Visit.

Step #1: New Patient Registration

Functional Medicine of Idaho uses an online portal to track all patient records, appointments, and communications. To become a patient at FMI, you will first create an account in the Patient Portal. Click the button below to create a new patient account.


After you’ve created an account, log in to your patient portal and complete all intake questionnaires located on the right hand side of the screen:

Step #2: Welcome Call

Once you’ve submitted all 11 intake forms in the patient portal, you will receive a Welcome Call from an FMI Patient Experience Navigator within 5 business days. 

During this 15 minute Welcome Call, a Patient Experience Navigator will help you register for the first Foundational Lifestyle Shared Medical Visit, schedule your Provider Intake Visit, address any missing information in your patient portal, and answer any questions you have.

Step #3: Functional Medicine as a Model for Healthcare Shared Medical Visit

The functional medicine model of care is not new, but is still uncommon in modern healthcare. We aim to change that. 

During the Functional Medicine as a Model for Healthcare Shared Medical Visit, we will take a deep dive into exploring the functional model of healthcare, learn why it’s so significant, and answer your questions about what you can expect on your functional medicine journey.

What to Expect During This Shared Medical Visit

This hour-long shared medical visit with a provider and a small group of new FMI patients will cover: 

  • The foundational principles of the functional medicine model
  • Why functional medicine matters
  • The Functional Medicine Tree
  • The model of care at Functional Medicine of Idaho 
  • An overview of the preliminary labs that will be ordered for you

Scheduling

The Functional Medicine as a Model for Healthcare Shared Medical Visit is offered in-person at our Meridian Wellness Center, and virtually, via Zoom, several times each week. During your Welcome Call (Step #2), a Patient Experience Navigator will help you register for a session that works with your schedule.

Our in-network patients will pay a copay for the Functional Medicine as a Model for Healthcare Shared Medical Visit, and their insurance will be billed for CPT 99203.

For out-of-network patients, we offer a 15% cash-pay discount for this shared medical visit. We accept all HSA and FSA cards as a form of payment, and can provide you with a detailed receipt to submit directly to your insurance for reimbursement.

Step #4: Blood Panel + Preliminary Lab Review Appointments

Because functional medicine is focused on identifying the underlying cause of disease, we utilize specific lab tests to gain a better understanding of what exactly is going on inside your body.  

We highly recommend completing a preliminary blood panel before your Provider Intake Visit. You will have your blood drawn to run a panel that includes 6 basic tests. Once your results are processed (typically within  2 – 3 weeks), you will have a dedicated 30 minute lab review with a provider to walk through your blood panel results and begin to formulate a plan to address any imbalances.

Scheduling

If you’re interested in completing a preliminary blood panel, a Patient Experience Navigator will schedule a blood draw appointment and a lab review session that work with your schedule. The lab review is offered virtually, via Zoom, and may be completed at any time before your Provider Intake Visit.

Step #5: Foundational Nutrition Shared Medical Visit

Food is not just calories; it is information that can alter your gene expression, causing or preventing disease. Functional medicine harnesses the transformational power of food as medicine. The right nutrition, combined with a healthy lifestyle modifications, will help you transform your health.

During the Foundational Nutrition Shared Medical Visit, you’ll be introduced to your nutritionist, learn about the role food plays in your long-term health and well-being, be introduced to a Foundational Food Plan, and be able to ask questions about what to expect while working with a nutritionist.

What to Expect During This Shared Medical Visit

This hour-long shared medical visit with a nutritionist and a small group of new FMI patients will cover: 

  • How food changes the expression of your DNA through epigenetics
  • Using food to detoxify your body
  • What food toxins are and how to identify toxic symptoms
  • What phytonutrients are and why we need them 
  • The FMI Foundational Food Plan + simple meal ideas
  • An overview of the recommended Gentle Detox Supplement Kit

After your Functional Medicine as a Model for Healthcare Shared Medical Visit (Step #3), a Patient Experience Navigator will help you register for a  Foundational Nutrition Shared Medical Visit that works with your schedule.

The cost for the Gentle Detox Supplement Kit is $136.35 for all patients. We offer a 10% discount on this supplement kit for new patients.

Step #6: Foundational Health Coaching Shared Medical Visit

The key to experiencing success within the functional medicine model is to implement the lifestyle modifications that are recommended by your nutritionists and providers. Our patients work closely with health coaches to navigate the modifiable lifestyle factors that are part of a prescription towards greater health.

During the Foundational Health Coaching Shared Medical Visit, you’ll be introduced to your health coach, learn why health coaching is an imperative element of the functional medicine process, and be able to ask questions about what to expect in your coaching sessions.

What to Expect During This Shared Medical Visit

This hour-long shared medical visit with a health coach and a small group of new FMI patients will cover: 

  • What health coaching is and how it can benefit you on your wellness journey
  • How lifestyle affects your health
  • The power of setting SMART goals
  • The stages of behavior change
  • Support tools for new habit implementation

The Foundational Health Coaching Shared Medical Visit is offered in-person, at our Meridian Wellness Center, and virtually, via Zoom, several times each week. After the Foundational Nutrition Shared Medical Visit (Step #5), a Patient Experience Navigator will help you register for a Foundational Health Coaching Shared Medical Visit that works with your schedule.

Billing Information

Many health insurance companies do not offer coverage for health coaching appointments. Because of our commitment to offer health coaching to our patients, this service is considered “cash pay” at Functional Medicine of Idaho. The cost for the Foundational Health Coaching Shared Medical Visit is $60 for all patients. 

We can provide you with a detailed receipt to submit directly to your insurance for reimbursement.

Step #7: Provider Intake Visit

Functional medicine supports the idea that you, along with your provider, are a co-creator of your health. During the Provider Intake Visit, you will meet with your provider, have the opportunity to discuss your health concerns, and formulate a treatment plan together. You can expect to receive evidence-based, functional healthcare that is rooted in the latest research in modern medicine and tailored to your individual needs.

What to Expect During Your Provider Intake Visit

This hour-long intake visit with your provider will include:

  • A discussion about your symptoms, concerns, and health goals
  • A review of your detailed medical history
  • A review of the lifestyle changes that you have implemented throughout the Foundational Lifestyle Program
  • The development of an in-depth treatment plan to address the root cause of your illness and help you reach your health goals

What makes a patient a good fit for FMI?

Functional medicine emphasizes nutrition and lifestyle treatments to address the underlying issues that lead to chronic illness. All new patients at Functional Medicine of Idaho should be prepared to work with nutritionists and health coaches, in addition to their provider, to implement the nutritional and lifestyle modifications required to reach optimal health.

Do I have to complete all of the steps?

This program is an optional offering that allows our patients to jump start their health journey prior to their Provider Intake Visit. Completing the Foundational Lifestyle Program helps ensure a more efficient and effective intake with your provider. 

What is a shared medical visit?

Shared medical visits (SMVs) are hour-long sessions where patients are seen by a provider, nutritionist, or health coach in an educational, interactive, group setting.

How many people will be in my shared medical visits?

Shared medical visits can vary in size, but no more than 10 new patients will be in a session.

Does FMI still accept primary care patients?

We highly recommend keeping your current primary care physician until after your Provider Intake Visit, in the event that any emergent health issues arise or if you need any prescriptions refilled. After your Primary Intake Visit, you may consider FMI to be your primary care provider (PCP). 

How long should I keep my primary care doctor?

We highly recommend keeping your current primary care physician until after your Provider Intake Visit (Step #7), in the event that any emergent health issues arise or if you need any prescriptions refilled. After your Primary Intake Visit, you may consider FMI to be your primary care provider (PCP).

When does my insurance get billed?

Each visit will be billed to your insurance directly after you’ve completed it.
