It is best if you contact your insurance provider to make sure we are in network with your particular plan. We are in network with the following insurance plans, however this does not guarantee coverage:
Blue Cross – Regence – Aetna – Ameriben – Mountain Health CoOp
We accept most insurances as out of network. We are no longer accepting new Medicare and Tricare patients. Dr. Eberharter can see Medicare patients on a self-pay basis with the understanding that she cannot bill to Medicare as she is not contracted with them.
It is your responsibility to know and understand your copay and deductible information. Copayment is due at the time of service.
We do accept new patients on a limited basis. We are not able to accept new Medicare or Medicaid patients at this time. Our staff will help you assess if our practice is right for you and the time frame it might take for us to see you. We will do our best to suggest an alternative if we cannot see you.
We have an e-newsletter which is sent out on a monthly basis. We use this to alert you of any upcoming classes by anyone at the clinic and provide you with various health and lifestyle articles. Please feel free to contact us or inquire at the front counter in our office. You can unsubscribe at any time.
